The temporomandibular joint (TMJ) is the most-used joint in your body — it works with every word, bite and yawn. When it malfunctions, the symptoms are surprisingly varied and often misattributed.
Symptoms Beyond "Jaw Pain"
- Clicking, popping or grating sounds when opening
- Jaw locking open or closed — even briefly
- Pain radiating to the ear, temple or neck — frequently misdiagnosed as ear problems
- Morning headaches or facial muscle fatigue
- Difficulty or pain chewing hard food
- Teeth that suddenly feel like they don't fit together right
What Causes It
- Clenching/grinding (bruxism) — the dominant cause; stress-driven, often during sleep
- Bite problems — missing teeth or poor alignment overloading the joint
- Trauma — impact, whiplash, or prolonged wide opening (long dental visits, intubation)
- Arthritis — the joint can develop osteoarthritis like any other
- Posture — forward-head posture strains the same muscle chain
Conservative Treatment (First Line — Works for Most)
- Soft diet for 2–6 weeks — give the joint rest; no gum, hard or chewy food
- Night guard (occlusal splint) — a custom guard unclenches the bite during sleep; the single most effective intervention for grinders
- Physiotherapy — jaw exercises, posture correction, ultrasound therapy
- Stress management — clenching tracks stress directly; brief daytime awareness ("lips together, teeth apart") retrains the habit
- Warm compresses + short-course anti-inflammatories for flare-ups
When It Needs More
- Arthrocentesis — joint lavage for stuck or inflamed joints
- Botox into masseter muscles — for severe clenching unresponsive to guards
- Surgery — genuinely rare, reserved for structural problems like disc displacement or tumours
The Good News
Most TMJ disorders are muscular, self-limiting and resolve within weeks to months of conservative care. Persistent clicking without pain usually needs monitoring, not surgery.
